Registration Information Survey 2020-2021 Season
The PCMHA Board is working on our Return to Ice plan, but we need your help. We are asking you to fill out this 2 question survey so we are able to get a better understanding of how many players we will have for the 2020-2021 Season.Attached is the link to a 2 question survey. We are just asking if you going to be registering a player this year for the season and what the date of birth is for that player.
If you have more than 1 child, you can just select done and by clicking the link again enter for the other players.
If you have any issue with the link please send an email to me at communication@pcmha.com
*This is a free version of survey Monkey so there is not as many options for the survey. When filling out the survey you can just type the date into the date field and hit the Done button.
